Language to Include a Gift in your Will or Trust
A simple, flexible, and versatile way to help Baystate Health attract the best physicians and care teams, provide the latest technology and treatments, and make a meaningful impact on the delivery of high quality care is through a gift in your willor living trust, known as a charitable bequest.
A specific bequest is one of the more popular ways to make a charitable gift by will. You simply leave a specified dollar amount or % of your estate to Baystate Health.
Example: I give DOLLAR AMOUNT [or ___% of my estate] to the Baystate Health Foundation, Inc., a charitable organization established by law in Massachusetts (Federal Tax ID# 04-3549011), to be used for _______________________("Baystate Health's greatest needs" or name of hospital or program).
You may also direct your gift to a favorite hospital, program, or area you would like to support at Baystate Health.
Example: I give DOLLAR AMOUNT [or percentage of my estate] to the Baystate Health Foundation, Inc., a charitable organization established by law in Massachusetts (Federal Tax ID# 04-359011), to be used to fulfill the mission of Baystate Health, with a focus on the following purpose: [State the purpose as broadly as possible].
